Adult Application

Adults wishing to join the Pack should complete the Adult Application.

Parent Talent Survey

This form is needed so your Pack and Den leaders can assess parent talents and determine who might be best suited to help with various activities. Help from parents is vital to the success of scouting and is greatly appreciated. Forms can be turned in at Den or Pack meetings.

 

Pack 809 Code of Conduct

Pack 809 has established the following Code of Conduct to provide Scouts, Parents, and Adult Leaders with a framework of expectations and disciplinary actions for all members of the Pack. This Code of Conduct will help ensure a common language and understanding for behavioral expectations, and consequences, allowing us to create an atmosphere conducive to fun, safety and the values of Scouting.

Medical Form

Medical forms should be filled out annually for each scout.

Reimbursement Form

Use this form to request reimbursement for out of pocket expenses related to Den or Pack activities.

"Class B" T-Shirt Order Form

The scouts are given their first Class B T-shirt when they join the Pack, but as they grow they may need new shirts. Download this form to order a new Class B red t-shirt for your scout, cost is $15.

JUMC Church Use Form

This form must be completed and submitted to Jarrettsville United Methodist Church to request the use of their facilities for Den or Pack related meetings and activities.

Sexual Misconduct Questionnaire

All parents are required to complete this form. Please note - This form must be submitted on PINK PAPER so if printing from the website please use pink paper. Otherwise please request a copy of this form from a Den or Pack leader.
